What Is Depression?

Depression, also called major depressive disorder, is a mood disorder characterized by persistent feelings of sadness or a lack of interest in life. While occasional sadness is a natural response to challenges, depression involves intense, prolonged symptoms that interfere with daily functioning.

Symptoms of Depression

According to the DSM-5, depression is diagnosed when five or more of the following symptoms persist for at least two weeks:

  • Persistent sadness or low mood most of the day.
  • Fatigue or low energy nearly every day.
  • Feelings of worthlessness or excessive guilt.
  • Hopelessness or pessimism.
  • Difficulty concentrating or making decisions.
  • Sleep disturbances (insomnia or oversleeping).
  • Loss of interest or pleasure in most activities.
  • Thoughts of death or suicide.
  • Restlessness or a sense of being slowed down.
  • Significant weight changes or changes in appetite.

Other symptoms might include irritability, physical pain (e.g., headaches, digestive issues), or mood changes tied to specific events like seasonal shifts.

Causes of Depression

Depression is complex, often resulting from a combination of factors, including:

  • Brain Chemistry and Structure: Imbalances in neurotransmitters like serotonin and norepinephrine can affect mood regulation.
  • Hormonal Changes: Life events like pregnancy, menopause, or thyroid issues can trigger symptoms.
  • Genetics: A family history of depression increases your likelihood of developing it.
  • Environmental Stressors: Trauma, loss, or prolonged stress can contribute to its onset.

Types of Depression

Several forms of depression exist, such as:

  • Major Depression: Severe symptoms that interfere with daily life.
  • Persistent Depressive Disorder (Dysthymia): Chronic depression lasting at least two years.
  • Seasonal Affective Disorder (SAD): Symptoms tied to seasonal changes, typically worsening in winter.
  • Peripartum Depression: Occurs during pregnancy or after childbirth.
  • Atypical Depression: Includes symptoms like mood improvement after positive events and increased appetite or sleep.
  • Depression With Psychotic Features: Involves delusions or hallucinations.

Diagnosis and Treatment

A depression diagnosis typically involves:

  • Physical Exams and Lab Tests: To rule out other medical conditions.
  • Psychiatric Evaluations: To explore thoughts, feelings, and behaviors.
  • DSM-5 Criteria: To confirm the presence of depressive symptoms.

Treatment Options

  1. Medications:
    • Antidepressants like SSRIs or SNRIs.
    • Additional medications for mood stabilization or anxiety.
  2. Therapy:
    • Cognitive Behavioral Therapy (CBT) helps reframe negative thinking.
    • Talk therapy provides a supportive space for emotional exploration.
  3. Brain Stimulation Therapies:
    • Electroconvulsive Therapy (ECT) or Transcranial Magnetic Stimulation (TMS) for treatment-resistant cases.
  4. Lifestyle Adjustments:
    • Regular exercise, proper sleep, and mindfulness practices.
  5. Crisis Intervention: Seek immediate help for suicidal thoughts through hotlines or emergency services.

While there is no permanent “cure,” effective treatment can lead to remission, allowing individuals to lead fulfilling lives. Early intervention and ongoing care are crucial for managing symptoms and preventing recurrence.
